Will I need to contact my doctor or health care provider to get copies of my medical records or reports?

What if the judge sends me a form for my doctor to fill out?
Sometimes the administrative law judge will want your doctor to fill out a form about his or her opinion on your ability to work in spite of your medical condition. Are there any documents my lawyer doesn’t need to see?
